Our Caregiver Solutions series of monthly events are provided through a grant from Direction Home Area Agency on Aging and Disabilities. Caregivers can join us from the convenience of their homes on the second Tuesday of every month at 2 p.m. If you pre-register, you can ask questions through the webinar or join us here on the Vantage Aging Facebook Page, where the webinar will be shared live. Who joined us?
On June 13, 2023, Vantage Aging hosted Bill Adams from Heyday Health and Tony Stover from Grandpa’s House, LLC.
Heyday Health is in-home and telehealth primary care for those with Medicare, cofounded by Dr. Mehta, a Harvard-trained physician.
Heyday Health services combine the best features of the house calls of the past with the tools of current technology, including virtual visits. Being homebound is not required. Heyday accepts traditional Medicare, Medicare Advantage, and dual-eligible patients with both Medicare and Medicaid. They are unable to accept Medicaid only.
Grandpa’s House LLC was established in 2021 by Tony and Gina Stover. After Gina’s grandpa passed away and her grandma went to an assisted living facility, her family worked together to go through belongings and renovate the home. This sparked the creation of their business to help other families going through similar circumstances or other transitions.
About Heyday Health
At Heyday Health, patients have 24/7 access to their care team with the convenience of telehealth and house calls. The care team comprises of a physician, a nurse practitioner, and a Health Ally. The Health Ally is a new position that helps the patient navigate the health system, assisting with phone calls on the patient’s behalf, referrals, and connections with community resources.
Anything provided in a traditional physician’s office can be coordinated at home, as well as labs and X-rays. Prescriptions can be delivered. These features help patients access the care and services they need. After a visit, there are follow-up phone calls within a week.
The benefits of seeing patients in their home setting include staff assessments for safety and resources for that patient’s specific needs.
Patient portals and health journals are available for patients to keep in their homes. The health journal includes care team information and a summary of visits. This hard copy can be of benefit for patients to review.
